Preparing for Your First Visit
Getting a few things ready ahead of time can help your appointment run more smoothly. We recommend bringing:
- Government-issued photo identification
- Dental insurance information, when applicable
- A current medication list
- New patient paperwork, if you completed it in advance
- Relevant dental or medical records you may have
- A list of symptoms, concerns, or questions you'd like to discuss
New Patient Forms
Our digital patient forms allow us to learn more about you and your dental needs before you arrive at our office. Completing them in advance helps reduce paperwork at check-in and allows your appointment to get started sooner. Moving away from paper also helps minimize waste, reduce our environmental impact, and streamline office operations, allowing us to focus more of our resources on delivering quality dental care.

What Your First Visit May Look Like
Your initial appointment is designed to give us the information we need while giving you plenty of opportunity to ask questions and understand your care.

Check In and Get Settled
When you arrive, our team will help you complete any necessary paperwork and make sure you're comfortable. New patient forms can also be completed online in advance to make check-in easier.

Review Your Oral Health
We'll take a detailed look at your teeth, gums, bite, and overall oral health. When appropriate, diagnostic X-rays will help us identify concerns that may not be visible during a visual examination.

Talk Through Our Findings
Once your evaluation is complete, we'll explain anything that needs attention in straightforward terms. Our goal is to make sure you understand what we're seeing, why it matters, and what treatment options may be available.

Cleaning When It Makes Sense
A cleaning may be completed during your first visit depending on your oral health and the type of care you need. If deeper periodontal treatment is more appropriate, we'll explain that before moving forward.

Discuss Your Next Steps
We'll review any recommendations with you and help establish a plan based on your needs, priorities, and comfort level. You'll know what comes next before you leave the office.
Insurance and Financing
We believe financial questions should be addressed openly so you can make informed decisions about your care. Eagle Mountain Dentistry works with many major dental insurance plans, and patients without insurance may qualify for our $99 New Patient Special, which includes a cleaning, exam, X-rays, and a complimentary consultation or second opinion.
